AUTORIZACIÓN DONANTE
Al llenar este formulario autorizo al emisor para que a partir de la presente fecha cargue mensualmente (o un cobro único de ser especificado en comentarios) a la tarjeta de crédito/débito antes indicada la cantidad descrita. Me comprometo a informar por escrito en caso de cambio o cancelación. El cargo automático se hace a principio de mes entre los días 1 y 5 o a fin de mes entre los días 20 y 31.
El recibo se envía a través del correo electrónico, de ser necesario obtenerlo físicamente comunicarlo a Valle de los Ángeles.

By filling out this form, I authorize the issuer to charge the credit/debit card above on a monthly basis (or a one-time charge if specified in comments) the amount described above. I undertake to inform in writing in case of change or cancellation. The automatic charge is made at the beginning of the month between the 1st and 5th or at the end of the month between the 20th and 31st.
The receipt is sent through email, if necessary to obtain it physically, communicate it to Valle de los Angeles.
